Apply for the intermediate license in person at a DHSMV Driver License Service Center.Ask your driving supervisor (parent, guardian, or driving instructor) to fill out the form Certification of Driving Experience of a Minor (HSMV 71142) and sign it in the presence of a Driver License Service Center employee or Notary Public and bring it for your appointment.
